New AND Returning Student Registration Forms

Please complete the following forms for your New or Returning Student and return to FCMS. Once all mandatory forms are submitted and accepted, the registration process is complete!

Please note that your child will not be registered until all forms indicated as MANDATORY below are completed and returned. Please click all 3 mandatory links and complete and submit these forms.

Intent to Re-Enroll Form

The Intent to Re-Enroll form secures your currently enrolled child’s spot for the coming school year. An Intent to Re-Enroll form must be completed and submitted to the school office by end of day January 15, 2024.

Medical and Healthcare Forms

All health care plans/paperwork expire end of July each year and must be renewed going into the new school year. For children with special health needs, the following forms must be filled out in their entirety, and signed by both a legal guardian and a health care provider:



✓ If an medication is needed to have on hand/given at school – Authorization and Release for Administering Medicine at School
✓ Students with allergies requiring emergency medication/EpiPen – Colorado Allergy and Anaphylaxis Care Plan
✓ Students with asthma requiring as needed/emergency inhaler – Colorado Asthma Care Plan
✓ Students with seizures needing emergency medication at school – Seizure Emergency Care Plan
